The Case Aide assists the Case Manager with implementation of assigned functional aspects of the case management process which will enable the client to engage in services, support timely and complete referrals, build the community resource base, and enhance follow-up activities. The case aide will participate in data collection, computerized tracking and reporting on outcomes.
Principal responsibilities include:
• Provide assistance to clients as designated in the individual service plan.
• Prepare confidential information for referral to service providers.
• Visit community service providers to gather information regarding potential service provision.
• Conduct follow-up activities as assigned by supervisor.
• Gather and organize data for required reports.
• Assist in data entry for computerized client tracking.
Minimum qualifications:
High school diploma and one year of experience with similar populations.
